dc.description.abstractAn integrative review was conducted to understand, from the perspective of patients with anorexia nervosa (AN) and their families, which factors hinder or favor adherence to treatment. The protocol followed the six stages of Mendes, Silveira and Galvão (2008). The search was conducted in PubMed, ScienceDirect, SciELO and the Virtual Health Library (BVS), using MeSH/DeCS descriptors and exclusion and inclusion criteria. Fourteen studies met the criteria, totaling approximately 375 participants, including patients, parents, siblings and other caregivers. The thematic synthesis revealed five central barriers to adherence: invisibility of emotional suffering, parental burden in the initial phase of Family-Based Therapy, protocol rigidity, fragmented communication between services and abrupt transition to adult care. The facilitators included empathic therapeutic alliance, family strengthening through practical workshops, multi-family support networks, guilt-reducing neurobiological psychoeducation, and intensive daytime programs that alleviate the domestic burden. It is concluded that staying in treatment requires integration between evidence-based techniques and psychosocial support, with active listening to family singularities.pt_BR

dc.description.resumoRealizou-se uma revisão integrativa para compreender, na perspectiva de pacientes com anorexia nervosa (AN) e de seus familiares, quais fatores dificultam ou favorecem a adesão ao tratamento. O protocolo seguiu as seis etapas de Mendes, Silveira e Galvão (2008). A busca foi realizada no PubMed, ScienceDirect, SciELO e Biblioteca Virtual em Saúde (BVS), utilizando descritores MeSH/DeCS e critérios de exclusão e inclusão. Quatorze estudos preencheram os critérios, totalizando cerca de 375 participantes entre pacientes, pais, irmãos e outros cuidadores. A síntese temática revelou cinco barreiras centrais à adesão: invisibilidade do sofrimento emocional, sobrecarga parental na fase inicial da Terapia Baseada na Família, rigidez protocolar, comunicação fragmentada entre serviços e transição abrupta para a assistência adulta. Entre os facilitadores destacaram-se aliança terapêutica empática, fortalecimento familiar por meio de oficinas práticas, redes de apoio multifamiliares, psicoeducação neurobiológica redutora de culpa e programas diurnos intensivos que aliviam o fardo doméstico. Conclui-se que a permanência no tratamento exige integração entre técnica baseada em evidências e suporte psicossocial, com escuta ativa das singularidades familiares.pt_BR
